He outlines the current use of AI in mental health from low-risk administrative support (transcription, summaries, risk-language flags) to adjunctive tools (CBT exercises, mindfulness, psychoeducation, mood tracking) and, more controversially, AI positioned as a primary provider via chatbots and companion apps.
He reviews mixed research: digital CBT can help mild to moderate anxiety/depression (e.g., Woebot) but lacks credible evidence and is inappropriate as a primary intervention for complex conditions like trauma, severe depression, suicidality, psychosis, complicated grief, and relational trauma.
He highlights ethical concerns: inadequate informed consent, lack of scope-of-practice safeguards, crisis-response failures, and absence of a true therapeutic alliance. In this episode Dr. George Vergolias explains why “staying gone” after leaving toxic relationships, workplaces, or partnerships is harder than leaving, as guilt, doubt, and second-guessing surge.
He reframes walking away as choosing rather than quitting, and describes how toxic people and systems weaponize values like loyalty and compassion. He outlines the neuroscience of guilt and attachment (social-emotion circuitry, oxytocin bonding, intermittent reinforcement and dopamine), distinguishing appropriate guilt from boundary enforcement.
He differentiates healthy detachment from coldness, emphasizing compassion without emotional merging and noting detachment as a leadership competency. Using a real-world case example, he explains how guilt often reflects identity disruption. He argues that closure is largely a myth, often protest behavior is especially ineffective with high-conflict personalities, and that real closure is when we show internal acceptance and integration.
He provides a five-step framework:

narrow access without closing the heart
